Job Summary <br>
<br>
The position provides primary care, urgent care, occupational health, case management, and emergency services.  The Mid-level Practitioner provides the various categories of care as noted above, making independent clinical decisions with consultation and collaboration with PIMMS physicians.<br>
<br>
Ancillary position duties include supervision and scheduling of part time Mid Level Practitioners, formulary management, as well as some minor marketing duties with our corporate clients.<br>
<br>
Practice is M-F day shift with one weekend of call per month as well as some weekday call.<br>
<br>
 Essential Job Functions <br>
<br>
•	Management of chronic health problems in conjunction with PIMMS physician staff.<br>
•	Primary care for trauma patients, including suturing and other procedures.<br>
•	Perform simple CLIA waived laboratory tests and interpret laboratory test data.<br>
•	Conduct primary care site visits to patients on vessels in the Columbia River.<br>
•	Provide occupational health examinations and education to patients.<br>
•	Provide travel health consultation and vaccinations.<br>
•	Provide case management using PIMMS maritime paradigm of care.<br>
•	Candidates must be eligible for licensure in both Oregon and Washington.<br>
•	Candidates must be physically able to board ships, requiring the ability to climb stairs, etc with approximately 15lbs of medical equipment.<br>
<br>
Job Skills and/or Abilities <br>
<br>
•	Ability to perform medical examinations using standard medical procedures.<br>
•	Knowledge of patient care charting, patient histories, clinical operations and procedures, primary care principles and practices. <br>
•	Ability to react calmly and effectively in emergency situations.<br>
•	Ability to self-manage assigned patient caseload, including organizing, prioritizing, and scheduling appointments, services, and work assignments.<br>
•	Ability to problem solve complex situations and manage competing demands presented in similar clinical situations.<br>
•	Ability to make administrative and procedural decisions.<br>

Education and/or Experience <br>

<b>Assistant home managers will empower individuals with developmental/intellectual disabilities in a vocational setting to improve their independence and quality of life, and to help them live out the choices that they want to make!</b>

Staff will assist consumers in their daily activities, as well as, plan and conduct activities out in their communities. Staff will transport and accompany individuals to appointments using a company vehicle, as well as, assist with personal care and medication administration, social skills and learning. There is a fair amount of documentation involved, so good written communication is a must. 
<br>
You must be comfortable performing a wide variety of tasks and dealing with occasional behavioral challenges. Full training provided including non-confrontational restraint methods. <b>We have one of Oregon's top training programs.</b>
<br>
Overall operation and supervision of a vocational program while working closely with the Vocational Director, Behavior Specialist and Clinical Director; supervising and training employees, assuring proper procedures are followed; monitoring and thorough documentation of funds, maintenance, and all other aspects of care.

GENERAL SUMMARY<br>

Assist doctor in delivering quality health care to patients. Performs all duties in a manner which promotes team concept and reflects the standards of AADA.<br>

RESPONSIBILITIES<br>

1. Prepare, stock, and clean exams rooms. Set up exam rooms for special procedures. Set up allergy testing trays.<br>
2. Escort patients promptly to exam rooms and obtain necessary information for physician. Patient history, pulmonary function tests, medications, allergy testing, and give multiple types of injections.<br>
3. Obtain specimens from patients using appropriate technique, including phlebotomy, and cultures as directed by the physician. Provide telephone results to patients.<br>
4. Assist physician with procedures. Perform duties on diagnostic equipment such as Spirometry and Tympanogram.<br>
5. Provide patient education and instructions on medications, inhalers, peak flow meters, and wound care.<br>
6. Perform phone triage.<br>
7. Maintain high standards of technique per clinic protocol in performing all functions.<br>
8. Perform other related duties as assigned by the Physician.<br>
9. Coordinate team and supervise at least one other nurse.<br>

Our current, longstanding and beloved PMHNP is partially relocating and significantly cutting back hours to part-time; however, she will still be available to share in a flexible schedule, as well as call. Direct Support Staff provide direct client support services and are key to the organization’s success and the quality of services our clients receive.  In this position, Direct Support Staff assist our clients with the maintenance of their home, meal preparation, laundry, cleaning and other household chores and with getting ready for work or their day program.  These positions provide tremendous variety in day-to-day routines and tasks, require problem solving, good communication and teamwork.  The program is well established, has an excellent regulatory record, enjoys great relationships with neighbors and has stable, long-term employees.<br>

Schedule is Monday-Thursday, 10:45 PM to 8:30 AM.    This position requires fluency in written and spoken English and the ability to lift up to 50 pounds. The position is hourly and starting pay will be $9.00 – 9.25 depending on experience. Follows and ensures that Health Care staff follow all facility policies, procedures and practices including confidentiality, protection of residents’ rights and property.  Promotes positive understanding of all functions and activities of the Health Service Department. Homewatch CareGivers start at an hourly wage of $10.00 to $10.50, depending on experience and certifications (e.g., CNA) and have access to our health & welfare benefits: medical and dental plans, 401K, Section 125 for dependent care and medical expenses. We are also pleased to offer a generous service bonus paid ranging from $200-500 per year, depending on hours worked and time with the company. Part-time, variable hours depending on your availability and willingness to travel. We have shifts from 2 hours to live-in assignments, throughout Multnomah, Clackamas and Washington Counties. We will be flexible in working with you to assign hours that fit your schedule – even if you have other employment commitments! Some of our clients are difficult to get to by public transportation, and many of our clients need transportation services, so a car is necessary to ensure you will get the hours that you desire.

They must also be able to communicate effectively and function with minimal supervision. They must be able to communicate CLEARLY to elderly persons who might have hearing problems. To be able to provide transportation services, Caregivers must possess a valid driver’s license, current automobile insurance and have access to a vehicle.
