Behavioral Health Engagement Specialist – CDP/T – Posting #19-074

Major Duties:
The Engagement Specialist works under the supervision of the Program Director. The specialist contacts the patients that are enrolled or referred for services in SUD OP and assists them in engaging in treatment. The purpose of the contact is to identify barriers to client attending and addressing them to re-engage client, and if client cannot be re-engaged in treatment services, create Discharge/Transfer Planning for client
In providing these services. The individual performs the following duties independently:

• Contacts the clients referred by centralized scheduling that are not attending scheduled service appointments to confirm if they want services.
• Identify barriers to client attending and addressing them (i.e., different day, time, etc.)
• If client cannot be re-engaged in treatment services, create Discharge/Transfer Planning for client
• .
• Assess and evaluate client needs, strengths, goals and resources.
• Refer and link clients to other human services programs after a series of continued late cancels or client no shows for appointment.
• Work with interdisciplinary team in reviewing cases and developing Individual Service Plans to reengage clients into services.
• Intervene, stabilize and manage acute crisis situations.
• Provide linkage case management and participate in treatment planning. .
• Consult with other agency staff regarding assessments and service plan development.
• Consult with other service agencies around case-specific and programmatic services.
• Other responsibilities and tasks as may from time to time be assigned.
• Maintain a minimum of 55% productivity.Maintain clinical and management records in accordance with assigned Services Programs.
• Maintain 100% UM/UR compliance.
• Other responsibilities and tasks as may from time to time be assigned.
• Conduct all professional activity in an ethical manner.
• Conform to all applicable policies and procedures.
• In conjunction with other staff assist in identifying and eliminating any situation which jeopardizes the health and/or safety of clients, staff, or others who come in contact with agency operations.
• Utilize literature, workshops and other training aids to continually improve clinical and administrative skills.
• In conjunction with supervisor, complete and implement Individual Professional Development Plan.
• Assist in assuring compliance with all related licensing and accreditation requirements.

Requirements:
• Meet credential requirements of a CDP or CDPT;
• Bachelor’s degree preferred;
• Experience in Behavioral Healthcare field with adults and/or youth preferred;
• Good verbal and written communication skills;
• Great attendance;
• Ability to work well with others.

Salary: DOE
